with large overhanging forehead; large projecting nose with black bottom; diamond-shaped mouth with one upper and lower tooth, four circles representing eyes separated by a horizontal band of triangles, five vertical incised lines at chin and three diagonal incised lines at each corner of the face, and a projection down from the chin; behind mask is an oval projection to fit the face, with a ring of holes for attaching the costume.

Inscribed: Label on back: '#509 -- TR-LA' (?). Painted on back of mask: 5077.
